We are sorry to hear that you wish to surrender your animal. Please take the time to complete this form, in its entirety, and we will add you to our waiting list and contact you when we have kennel space. If the form is not filled out completely, we will not be able to intake your animals or add your intake waiting list. 

Please be honest. We take in animals of all breeds, all ages and all behavioral or medical issues. Your honesty is crucial so we know how to best care for your pet. Thank you!
